Hawthorn House Admission Criteria
An individual must meet the following minimum criteria to be a resident at Hawthorn House:

Surviving spouse of military retiree, military couple, dependent of an active duty military member, or military retiree, age 55 or older.

Must be free of a communicable disease, which is likely to be transmitted to other residents or staff. Exception: A person who is Human Immunodeficiency Virus (HIV) positive may be admitted if otherwise eligible.

Must be able to perform everyday, basic activities necessary to daily life with assistance or supervision.

Must be able to transfer with assistance if necessary.

Must have dietary needs that can be met by the facility.

Must not be a danger to self or others.

Must not be bedridden.

Must not have Stage 2, 3 or 4 presurre sores.

Must not require 24-hour skilled nursing services or professional mental health care.

Must be deemed appropriate for admission by the Admissions Board and meet all other eligibility requirements.
Applying for Residency
The following guidelines are strictly followed in determining eligibility to live in the Hawthorn House, an Assisted Living community.
After your initial tour and/or a decision is made that you would like to move into the Hawthorn House, the following steps must be completed:
Step 1 Completion of the Department of Elder Affairs Form 1823. Nothing else can proceed until this form is filled out properly in it's entirety by the family doctor, to include a complete medication listing and all other information required by your health care provider. In the case of a couple, each individual will need to have completed 1823.
Step 2 Upon receipt of the completed 1823, an appointment will be made with the Director of Nursing or another Licensed Practical Nurse to further evaluate the applicant(s). This will determine your needs and level of care. The evaluation process will include:
Step 3 Upon completion of this evaluation, the Admissions Board will review all available information and observations to determine whether the applicant is appropriate for our Assisted Living Residence.
Step 4 If the applicant meets all of the regulatory admission requirements as well as the facility's admission criteria, and the facility is capable of providing the services required by the applicant, the admission process can proceed. It is at this point you will be notified of your acceptance to the Hawthorn House, and you can proceed with plans to schedule a move-in date and finalize the required paperwork necessary to make your transition as smooth as possible.
Step 5 An application must be completed and returned with the Admission Fee prior to move-in.
Step 6 Financial release authorizing credit check.
Step 7 Hawthorn House subsidy forms are available for those eligible applicants. These will be proc-essed by the Administrator or Admissions Coordinator.
Step 8 The Administrator will determine the level of care and payment plan.
Step 9 The Residency Agreement must be signed and a monthly fee must be paid in order to secure an apartment.
The resident will be responsible for fees as long as the unit is occupied. Absences, including vacation, hospitalization, nursing home placement, etc., does not release resident from fees.
